This form has the following fields:<br/>

<li>Payment Type ID – the internal ID of the Payment Type, read-only</li>
<li>Name – the name of the payment type</li>
<li>Primary – designates the payment type as primary</li>
<li>Enabled – the status of payment type. Only enabled payment types are displayed in the affiliate registration form on the front end</li>
<li>Priority – the priority value. The payment types are sorted according to the priority on the affiliate registration form on the front end</li>
<li>Description – the description of the payment type. This description is displayed below the name of the type on the affiliate registration form, on the front-end</li>
